Eastern Submission Forms
Please make sure you use the current forms. They should be labeled “Laurel v. 4.0; East v.1.0 (Laurel Approved 03 Jan 2023)” in the bottom right-hand corner. Forms labelled “Laurel v. 3.0; East v1.0 (Laurel Approved 28 JUN 2016)” are no longer acceptable, as the cutoff date was December 31, 2023.
Submissions on old or altered forms will generally be administratively returned.

The emblazon image files for device and badge are blank images that are sized to fit the related submission forms. If you’re creating digital art for submissions, you can use these images as a background when doing the artwork, save the final image, and click to import it into the emblazon area of the fillable form.
Note: importing the emblazon image files will cover a small amount of the instruction text in the lower corner of the form (Adobe does not support transparent image import). This is acceptable and is not grounds for administrative return.
